One Room Opening Available
We currently have one room opening in our coed home. We provide a comfortable living environment and professional services for individuals with intellectual disabilities, autism, diabetes, and dual diagnoses. A staff list is available upon request.

How Admissions Work
- Initial Inquiry
Call us or fill out our contact form. We’ll answer your questions and learn about your loved one’s needs.
- Tour Our Home
Schedule an in-person tour to meet our team and see our residence. Guardians and case managers are welcome.
- Assessment
We review the prospective resident’s support plan, medical history, and care preferences to confirm a good fit.
- Care Plan & Agreement
We finalize a personalized care plan with the resident, guardian, and APD support coordinator.
- Move-In Day
We welcome your loved one with a warm transition plan to help them settle in comfortably.
For Support Coordinators & Case Managers
We work closely with APD waiver support coordinators across Lee, Collier, and surrounding counties. We can provide:
- Vacancy and availability updates
- Service documentation and progress notes
- Open communication with the support team
